Health advice: Should men be screened for osteoporosis?

Osteoporosis screening in men is controversial, but a diagnosis should prompt consideration of the underlying reason behind it.

Dear Dr. Roach: My husband and I are confused about the guidelines concerning when older men should be screened for osteoporosis. All the information seems to suggest that it is only needed for men over 70 who have had a fracture. In my husband’s case, he was only told to get a DEXA (dual-energy X-ray absorptiometry) scan after he had back pain and several compression fractures of the thoracic spine (up to 70%) at the age of 72. His T-score was -2.8 for the spine.

I can’t help feeling that because he is male, weighs 200 pounds, has an active lifestyle and was, previously, six-foot-two, the issue of osteoporosis just never came up during his yearly primary care visits (even though his height had decreased since, at least, 2019, and he is now just six feet). This is a potentially life-threatening condition for older women and men, but the risks for women seem to get all the attention. What’s the reluctance to order a simple, fast, painless, not-too-expensive test to screen men earlier?

In general, screening tests (by definition, a test done on a person with no symptoms of the condition) are done when there is good evidence that the screening will lead to an improved outcome in a screened group compared with an unscreened group. The data in women is strong enough that all women over 65, and women younger than 65 with additional risks for osteoporosis, should be screened for osteoporosis with a DEXA scan, but there is not data strong enough to support screening in men.

Ready-to-wear brands tend to tap out at six-foot-two. Above that, you’re in the Big & Tall world, which can mean billowing cuts and erratic hem-lengths. But all is not lost when it comes to tall men’s clothing. This is how to make your extra inches add up.

5 Tips For Tall Men’s Fashion

1. Create Contrast

If you’re tall and slim, keeping things too simple only serves to make you look taller and slimmer. The best fix is to break things up, says Luke McDonald from men’s styling service Thread. “Avoid wearing one colour head-to-toe and use things like texture and pattern to break things up.”

2. Add Bulk

You want to add bulk, but you don’t want to disappear into your tall men’s clothes, so think in width. You’re all about the vertical axis, so look to clothes that create some sideways heft. Horizontal patterns will help – think Breton striped tops or colour-blocked tees – but so will chunky knits, puffer coats and structured tailoring.

N.S. Brotherhood Initiative provides health care to Black men


The Nova Scotia Brotherhood Initiative is a free program offering Black men access to health care in the Halifax community to improve their overall health and wellbeing.


As part of the initiative, a team of health-care professionals provide culturally-appropriate primary medical care, as well as health and wellness services, for men of African descent across the Halifax Regional Municipality.


According to Nova Scotia Health, the program’s health and wellness services offered include:


  • primary medical care

  • health promotion and wellness education

  • chronic disease management

  • navigation to community resources

  • variety of free health and wellness programs throughout the year


Dr. Ron Milne has worked with the Brotherhood since it started in 2014. Last week, he was honoured by the organization for his contribution and positive impact.


According to Milne, many Black men don’t access health care as often as they should, citing many factors including a lack of trust due to mistreatment in the past by the health-care system.


“Lack of providers who have cultural competence and understand the challenges and issues that Black men face. I think those are some of the issues,” he said. “And lack of providers who look like them, who they feel comfortable with.”


Milne says stigma also has a lot to do with the issue.


“Again, it’s that cultural competence factor coming into it, of having people who understand the issues, the history, the challenges,” said Milne.


As far as some of the medical issues that disproportionately impact Black men, Milne says there still isn’t a good library of data in Canada with that information.
